This survey template helps schools collect feedback from pupils in Years 6 to 12 on key aspects of school life. It covers catering, safety, classroom facilities and cleanliness, detailed evaluations of the canteen and library, school recommendation and open suggestions for improvement.
Table of contents
We now use LamaPoll, for example, to organise our all-day school programme and many other school activities, such as parents helping with sports day, surveys of children and parents, and internal evaluations. The intuitive usability and security standards convinced us, as did the accommodating pricing.
What is this template for?
The template is suitable for secondary schools wishing to collect structured feedback from pupils on the school as a whole. It can be used for internal evaluations, school development processes or recurring satisfaction surveys.
The questionnaire shows how pupils perceive catering, safety, classroom facilities, cleanliness, the canteen and the library. It also captures which areas matter most to them, whether they would recommend the school and which practical changes they expect.
What does the questionnaire cover?
The template combines an overall assessment of key school areas with more detailed questions about the canteen and library and open suggestions for improvement.
Example questions from the template
- Which year group are you in?
- How would you rate catering, safety, classroom facilities and cleanliness using grades from 1 to 5?
- Before use, explain whether 1 is the best and 5 the lowest rating.
- Which matters most to you: catering, safety, facilities or cleanliness?
- The library is evaluated later in detail but is missing from the existing priority question and should be added where appropriate.
- How satisfied are you with the canteen’s opening hours, choice, prices, waiting times, quality and taste?
- How important are these points to you?
- What should we improve?
- How satisfied are you with the library’s opening hours, choice and waiting times?
- How important are these points to you?
- Which further criteria should be evaluated?
- What should we improve?
- Would you recommend our school?
- What should we change so that you would definitely recommend our school?
- The existing response options corresponding to “probably yes” and “perhaps” should be differentiated more clearly before use.
From template to an effective pupil survey
Why pupil feedback on the school is useful
Adaptation, anonymity and data protection
Adapt the template to the specific school before use. Replace the name placeholder with a neutral form of address, correct the wording referring to the school as a whole, catering and safety in the school building or on the school grounds. Also replace “Further points” and the placeholder rows in the library section with genuine criteria or remove them.
The statement that the survey is anonymous should only be used where the settings, access rights and organisational process genuinely prevent responses from being linked to individuals. The email-address question at the end is particularly important: it should be technically separated from the survey responses or moved to a separate form so that anonymity is not compromised.
Explain the purpose, deadline, access to results and handling of open responses in advance. The template’s legal notice and privacy information must be replaced or reviewed by the school responsible for the survey.
