We offer a wide range of well-founded questionnaire templates that can be used for employee surveys, customer surveys, market research projects and evaluations in the healthcare sector, education and more. The range spans classic questionnaires and form templates from everyday business practice through to legally grounded templates and sample surveys used for specific occasions or in an academic context. The LamaPoll questionnaire portfolio can be divided into four fundamental categories:
1. Institutionally grounded questionnaire templates
Some of our templates are based on materials and concepts from established practice-based and research institutions. Our sample employee survey, for example, draws on templates from the Institut der deutschen Wirtschaft Köln (IW) and the RKW Kompetenzzentrum Eschborn, two organisations with many years of expertise in human resources management and organisational development.
2. Legally and normatively grounded templates
Another part of our portfolio is based directly on legal requirements and official recommendations for action. This includes, for example, our templates for the risk assessment of psychological stress, both in the workplace and when working from home. These are based on the requirements of Section 5 of the German Occupational Health and Safety Act (Arbeitsschutzgesetz) and on the methodological recommendations of the Federal Institute for Occupational Safety and Health (BAuA), and are therefore aligned directly with the statutory obligations of employers.
On our templates page you will also find questionnaires developed on the basis of legal regulations or new EU directives (materiality analysis, whistleblower form, supplier self-disclosure, and more). Because the legislative process in Germany follows a structured procedure and some EU regulations, for example, still have to be transposed into national law, the collection also includes questionnaires created on the basis of legislative proposals and draft bills.

3. Conceptually established questionnaires & forms

These were developed on the basis of many years of practical experience, sound methodology and established HR, market research and quality management standards.
